An overdose of heroin (especially if mixed with other CNS depressants such as alcohol or barbiturates) causes extreme respiratory depression, eventually leading to respitory and cardiac arrest and finally brain death.

Heroin and painkillers should be generally the same. In both cases, you would die from severe respiratory depression. With meth, death usually comes from a heart attack or stroke.
