I cannot take opioid pain medication because it makes me vomit. My sister is a heroin addict, so obviously she doesn't react to it the same way I do. How come two sisters from the same parents physically handle medications so differently?

Just because you two are sisters, it does not mean that you and her handle the same medications the same ways.

Because everyone is different. You have a very low tolerance for opiates, hence why it makes you vomit. Your sister does not. Some people just react to things differently.
